Summary: Students are required to submit an online Field Placement Verification Form (PVF) (<https://ssw.umich.edu/assets/field-student-placement-verification/> ) at the start of every term that the students are enrolled in the field education courses (SW 515 and SW 691).

Students are required to submit an online Field Placement Verification Form (PVF) (https://ssw.umich.edu/assets/field-student-placement-verification/ ) at the start of every term that the students are enrolled in the field education courses (SW 515 and SW 691). This form documents the final placement contact information, the days/hours the student will be dedicating to field education, an attestation that safety and insurance policies were reviewed, and confirms the number of credits.

Once submitted by the student, the field instructor/task supervisor reviews and signs the form and then it is verified by the Office of Field Education. Students will be contacted should any revisions be required to resolve discrepancies. The Field Placement Verification Form is approved by the Field Office when the student and field instructor have reviewed and signed and all information is verified as accurate for that enrolled term of field.

This form is a required course assignment, and is necessary for students to complete to obtain access to their educational agreement document each term students are enrolled in field education. Should students fail to submit this form by the established due dates, their field hours may be suspended until the form is verified complete, which may impact their ability to complete field hours as well as their field education grade.
